Reports To: RN Case Manager / Clinical Supervisor
Work Location: Client’s Home

JOB SUMMARY

Provides practical nursing care to injured workers within their scope of practice and supports the RN in delivering nursing interventions as outlined in the care plan.

QUALIFICATIONS

  • Active registration as an LPN with the BCCNM.
  • At least 1 year of practical nursing experience in community/home care.
  • Up-to-date CPR/First Aid certification.
  • Clear criminal record check under CRRA.
  • Proficient with documentation and use of clinical systems.

Responsibilities
  • Carry out client assessments and implement delegated care plans.
  • Provide wound care, administer medications, and perform catheter care.
  • Support HCAs by supervising delegated tasks and ensuring quality care.
  • Monitor and report changes in client’s health condition to the RN or Primary Care Practitioner.
  • Assist in client and caregiver education.
  • Maintain accurate documentation as required by Designated authority.
  • Participate in infection control and safety procedures.
  • Respond appropriately to emergencies and critical incidents.
  • Submit service reports and invoices via the Designated authority Provider Portal.
