We are seeking highly qualified, part- and full- time Licensed Practical Nurses (LPN) to join our team to deliver excellent care for our residents. The ideal candidates will have senior living experience (e.g., long-term care, assisted living, memory support) with a passion for working with an elderly population. Current shifts available: 3:00 PM - 11:00 PM (Part-Time and Full-Time) Key Responsibilities: Provide direct nursing care to residents according to established care plans, promoting physical, emotional, and personal well-being. Conduct assessments, document observations, and report changes in condition to nursing and physician staff. Monitor vital signs, weights, and blood glucose; administer medications, injections, and authorized treatments. Perform restorative nursing interventions, including positioning, ADL training, and passive range of motion exercises. Assist residents with transfers, ambulation, and daily activities (bathing, dressing, toileting, oral care) using proper safety techniques. Support participation in recreational, therapeutic, and social activities. Assist with resident admissions, transfers, and discharges. Collaborate with interdisciplinary care teams to develop, update, and implement individualized care plans. Educate residents and families on health management, disease prevention, and care practices. Ensure compliance with infection control, safety protocols, and facility policies. Document care accurately in electronic health records (EHR) or charts. Provide mentorship and guidance to nursing assistants and support staff as needed. Requirements High school diploma or GED required. Current Virginia LPN license. Current Adult CPR and BLS/AED certification. Minimum of three (3) years progressive experience in a CCRC, long-term care, home health, medical case management, or related medical setting. Demonstrated ability to serve as a positive role model for service excellence, prioritizing resident safety and high-quality nursing care. Excellent verbal and written communication, organizational, time management, critical thinking, and multitasking skills. High self-initiative with the ability to follow verbal and written instructions accurately and timely. Skilled in providing care to elderly residents with courtesy, patience, compassion, and professionalism. Upholds residents’ rights and maintains integrity and discretion in handling personal and health information. Knowledge of safety protocols, medical/nursing practices, laws, regulations, and guidelines; experienced in clinical and diagnostic procedures. Able to work independently, exercise sound nursing judgment, and provide hands-on or urgent nursing care as needed. Proficient with Electronic Medical Records (EMR), Microsoft Office Suite, and other computer/office equipment. Adheres to clinic schedules, deadlines, and reporting requirements.
