Location: Western & Central Ohio  Schedule: Monday–Friday, 8:00 AM – 4:30 PM  Weekend Rotation: Every 4th weekend (Friday & Monday off after worked weekend)  Holiday Rotation: 2 holidays per year  Hours: Full-time (40/week) Summary of Position: The Licensed Practical Nurse (LPN) delivers high-quality, compassionate direct patient care in the home, community, and care-center settings. This role supports patients and families through skilled symptom management, emotional support, and exceptional service aligned with the organization’s mission of providing dignified, patient-centered hospice care. Key Responsibilities: * Provide compassionate, high-quality hospice and palliative nursing care through home visits, inpatient care, end-of-life/continuous care, and admissions support * Deliver care in accordance with individualized Plans of Care and physician orders * Monitor patient condition and promptly communicate changes to the supervising RN * Educate patients and families on hospice services, insurance benefits, and care expectations * Coordinate care transitions and order necessary Durable Medical Equipment (DME) * Obtain and accurately document physician orders in the EMR * Complete timely, compliant documentation per agency and regulatory standards * Collaborate with interdisciplinary team members to ensure seamless, patient-centered care * Participate in evening and weekend rotations as scheduled * Perform additional duties as assigned to support patient care and team operations Qualifications  * Current Ohio LPN license * Minimum 2 years LPN experience (Med/Surg, Oncology, or Community Health; hospice preferred) * Valid driver’s license and reliable transportation * Strong communication and time management skills * Ability to work independently  Pay & Benefits  * Medical, dental, and vision eligibility 1st of the month following hire * 30 paid days off per year (PTO, sick time, and holidays) * 401(k) eligibility 1st of the month following hire * Employer-paid short-term disability * Tuition reimbursement * Incentives and shift differentials * Mileage & cell phone reimbursement * Monthly employee recognition events * PayActiv —early access to earned wages * PSLF-qualified employer * $200 initial clothing allowance for full-time positions Monday - Friday 8:00 AM - 4:30 PM
