Overtime Status: 12+ hours in 1 day or 40+ hours in 1 work week Summary The Licensed Practical Nurse (LPN) works under the direction of a primary care provider, as part of the healthcare team, and is responsible for providing general clinical nursing support. The individual is accountable for assessing, planning, implementing and evaluating the nursing care required for the patient and will assist in the development and revision of patient’s care plan. Job Description Prepares instruments, equipment, materials and exam rooms to ensure the appropriate items are available for patient care. Obtains and documents complete patient data in a timely and accurate manner to ensure the accuracy of the patient record for future use. Assists the provider with exams, tests and procedures and provides patient instructions to provide appropriate patient care and ensures the patient is knowledgeable on self-care procedures. Performs basic procedures as ordered by provider and within scope of practice to. Completes clerical or insurance processes to ensure the proper documentation and submission of charges is completed in a timely manner. Assists the provider with obtaining, processing or explaining test results or prescriptions to the patient to ensure the information is accurate and understood by the patient. Communicates with patients regarding their concerns and collects information to resolve the matter, if within their scope, or relays the information to the provider for further follow-up. Participates in the development of the care plan with other team members to safely and accurately implement the plan. Provides instruction to the patient and/or family members to ensure an understanding of the information pertaining to the patient. Completes steps to fill prescriptions per protocol and completes prior authorization forms to ensure proper billing and fill of the prescription for the patient. Regular attendance is required in order to carry out the essential functions of the position. Reviews and meets ongoing competency requirements of the role to maintain the skills, knowledge and abilities to perform, within scope, role specific functions. Other duties as assigned. Qualifications Graduation from an accredited Licensed Practical Nurse (LPN) program. For Wound Clinic, must complete a UHMS certified introductory course in wound care and hyperbaric medicine within first 3 months. Experience as a licensed practical nurse within an area relative to the hiring department preferred. Licensed Practical Nurse license awarded by the State of Michigan. Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within 30 days of hire. For LPNs who may be requested to float to a Wisconsin clinic, a WI LPN license must be obtained within the first 3 months. For Wound Clinic, a Certified Wound Care Associate (CWCA) certification is required within 3 ½ years. If travel to offsite locations is required, must possess a valid driver’s license and have a driving record without a significant history of accidents and/or convictions of moving violations. Responsibilities
The Licensed Practical Nurse provides general clinical nursing support under the direction of a primary care provider, being accountable for assessing, planning, implementing, and evaluating the nursing care required for the patient. Responsibilities include preparing exam rooms, obtaining and documenting patient data, assisting providers with exams and procedures, and providing patient instructions regarding self-care.
