Description Job Description Job Title: Licensed Practical Nurse (LPN) Department: Nursing Reports to: Director of Nursing FLSA Status: Non-Exempt Prepared Date: 11-1-17 Revised Date: 02-11-26 SUMMARY Responsible for providing direct nursing care as ordered by Physician, outlined in the plan of care and in accordance with standards of nursing practices and regulations. Takes charge in leading and directing resident/patient care on assigned unit. Directly supervises Certified Nursing Assistants to ensure assignments and care are complete based on professional standards. ESSENTIAL DUTIES AND RESPONSIBILITIES includes the following: • Visible commitment to the CES mission, vision, and values; emphasizing and modelling the need for collaboration and teamwork. • Personally participates i the evaluation and delivery of care including medication administration, treatments, Adl care, etc. As ordered by physician and outlined in plan of care (According to scope of practice) • Conduct regular rounds to monitor resident activity and ensure resident/patients receive quality care based on professional standards and facility policies • Evaluate residents physical and psychosocial status • Communicate with residents, family members, physicians and supervisor regarding resident status • Maintain up to date and accurate medical records during tour of duty on assigned unit • Coordinate and prepare for new admissions, transfers, discharge of residents • Transcribe physicians orders • Assist in updating the plan of care that addresses the needs of the residents/patients • Complete incident/accident reports • Understands, complies with and promotes all rules regarding resident rights • Maintain safe and clean work area • Ensures proper storage of equipment and supplies
Responsibilities
The Licensed Practical Nurse is responsible for providing direct nursing care, administering medications, and monitoring the physical and psychosocial status of residents. They also supervise Certified Nursing Assistants and ensure that all medical records and care plans are accurately maintained.
