Pinnacle Pharmacy, located in Nanaimo BC, is expanding services and actively seeking LPNs to join our growing team of dedicated healthcare professionals. Our Pinnacle Plus program provides enhanced access to nursing resources for clients within their home. With medication adherence and management being a core focus, our pharmacy led program provides our clients with the medication support, wellness monitoring and nursing care they need to continue living independently. We are looking for LPNs who are caring, friendly and who enjoy building relationships with their patients.
This position is day shifts only - no evenings or nights! Ideally suited for someone with strong leadership skills, ability to communicate effectively (both written and verbal) and the capacity to work both independently and within a closely connected team. We are seeking nurses who want to be actively engaged in patient health outcomes and who are enthusiastic about the positive contributions they will make in the well-being of the clients and their families.

JOB DESCRIPTION

  • In home nursing care, which involves administering medications (injections, oral medications, patches, inhalers, creams, eye drops) to clients in their homes
  • Individualized patient monitoring (i.e. blood glucose, blood pressure, pain assessments) and working with pharmacy team to apply those assessments as required for medication or care plan changes
  • Collaborating with pharmacists, physicians, nurse practitioners and other members of healthcare team
  • Therapeutic decision making within LPN scope in accordance with the pharmacy’s policies and applicable legislation
  • Noting and documenting changes in health status
  • Communicating daily reports to pharmacy team
  • Liaising with client’s family members

REQUIREMENTS

  • Licensed Practical Nurse, in good standing with BCCNM
  • Driver’s License (company vehicle provided)
  • Criminal Record Check upon accepted offer
  • Valid CPR-C and First Aid certificate

Hours:

  • Daytime shifts (8 hours), Monday through Sunday
  • Fair rotation of weekend shifts
  • Full time and part-time positions available
  • Please note this is not a “Travel LPN” position, we are searching for people to join our committed team

Experience:

  • Nursing: experienced nurses and new grads welcomed
  • Experience with diabetes management considered an asset
