We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. SKILLS

• Nursing home experience is highly desirable.
• Hospital experience in acute care settings is preferred.
• Familiarity with dementia care practices.
• Experience in pediatrics or home care settings is a plus.
• Proficient in patient care techniques and nursing fundamentals.
• Knowledge of medical terminology essential for effective communication.
• Competence in medication administration protocols.
• Experience with electronic health record systems such as Epic or Cerner is advantageous.
• Background in medical office operations or assisted living environments is beneficial.
• Understanding of senior care principles and hospice care practices.
• Laboratory experience related to patient diagnostics is a plus.
• Skills in pain management strategies are valuable.

Work Location: In perso

• Provide direct patient care under the supervision of registered nurses and physicians.
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
• Monitor patients’ vital signs and report any changes to the healthcare team.
• Assist with daily living activities such as bathing, dressing, and feeding.
• Maintain accurate patient records and documentation using electronic health record systems like Epic or Cerner.
• Support patients with dementia, pediatrics, senior care, hospice care, and pain management needs.
• Collaborate with interdisciplinary teams for effective case management and patient-centered care.
• Conduct phlebotomy procedures and laboratory tests as required.
• Educate patients and their families about health conditions and treatment plans.
