Key Responsibilities: * Perform wound assessments, assist the nursing team in developing and implementing treatment plans, and provide follow-up care in clients' homes, adhering to best practice standards. * Utilize wound care tools and supplies provided. * Use Personal Protective Equipment (PPE) provided. Qualifications: * Current LPN license in British Columbia with a valid registration with the BCCNM. * Valid First Aid and CPR certification. * Completion of Criminal Record Check with a vulnerable sector clearance. * Possession of a valid driver's license and reliable transportation within the Greater Vancouver area. * Experience with wound care is an asset but not required. Willingness to learn and develop wound care skills is ideal. * Strong communication, problem-solving, and critical thinking skills. * Ability to establish and maintain effective working relationships with healthcare professionals, client’s family members, and relevant organizations to meet clients' best interests and goals. * Great technical skills to ensure accurate and timely completion of client care documentation using our online systems.
