Employee Role: Private Duty Nurse (PDN)-LPN The Private Duty Nurse (PDN)-LPN is a member of the client care team that provides services directly, or acts as a placement agency, to deliver skilled nursing services to clients in their personal residences. The PDN LPN works under the supervision of the Clinical Manager. Reporting To: Clinical Manager Hour Classification: Non-exempt Scheduling: Flexible hours, Short 1-2 hrs shifts, 8 hrs shifts, 12hrs. Essential Job Responsibilities: Evaluates and treats clients - adult and pediatric, with appropriate level of skill according to client’s needs under the supervision of an RN, Clinical Manager. Assist with providing continuity of care and emotional needs of client and family. Medication Administration. Verbal demonstration of client rights and responsibilities. Recognizes and copes with emergencies appropriately. Observes the client’s general physical, emotional, and mental status, and reports all changes in the client’s conditions, family situation, incident, and occurrences during shift immediately to agency RN/CM or Customer Care Coordinator. Build relationships with clients to encourage compliance with care plans and client safety. Manage client/contract satisfaction with the highest level of customer service standards, communication, and documentation. Adheres to infection control policies and exposure control plan as mandated. Monitor and maintain clinical report, including Infection and Occurrence/Complaint Reports. Adheres to and promotes safety and injury prevention measures in client and workplace settings. Participate in community outreach activities that promote the goals and objectives of the agency. Maintain Confidentiality in all professional areas and consider the needs of clients and their families, other employees, physicians, visitors, and all members of the community in an informative, courteous, and compassionate manner. Ensure all HIPAA regulations are adhered to. All other duties as assigned. Routinely required to use hands for various tasks, including hand controls, gripping, and handling of objects. At times, will be required to reach beyond mid-line of the body, reach overhead or at chest level. All employees will be required to use universal safety precautions when performing their assigned tasks. Employees must have the ability to lift, push, pull and carry a minimum of 25 pounds. Employees must have the ability to stoop and bend up to 2.5 hours per 8-hour shift. Requirements Qualifications: Education: Graduate of an accredited school of nursing. Licensure / Certification: Licensed practical nurse (LPN) who is licensed by the state of Michigan. Must have a current, unencumbered license as an LPN, or an Advanced Practice Nurse. Must maintain all state and federal compliance requirements for licensure and employment. Travel: Travel is required for business purposes. Employees must have a valid driver’s license issued by the state in which they work and a satisfactory driving record. Work Experience: Minimum of one year of home care experience. One additional year of nursing experience, preferably in a hospital, is required for conducting applicable training classes. Other Hours of Work: Varies on a case-by-case basis. Full time is at least 30 hours a week from Saturday-Friday. Skills and Additional Requirements: Must be customer service oriented and prepared to liaise with patients, their families and other care providers. Satisfactory verbal and written communication skills. Ability to work well under pressure.
Responsibilities
