JOB SUMMARY

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have experience in a variety of settings, including nursing homes, hospitals, and home care. REQUIREMENTS

  • Valid LPN license in the state of practice.
  • Nursing home experience preferred; hospital experience is a plus.
  • Familiarity with acute care, dementia care, pediatrics, hospice care, and senior care practices.
  • Proficiency in medication administration and understanding of medical terminology.
  • Experience with electronic health records systems (Epic or Cerner) is desirable.
  • Strong interpersonal skills to effectively communicate with patients, families, and team members.
  • Ability to work collaboratively within a multidisciplinary team environment.
  • Previous experience in home care or assisted living settings is advantageous.

Responsibilities
  • Provide direct patient care under the supervision of registered nurses and physicians.
  • Administer medications and monitor patients for adverse reactions.
  • Assist with daily living activities for patients in various settings such as nursing homes, assisted living facilities, and home care environments.
  • Conduct assessments and document patient information accurately using medical terminology.
  • Collaborate with healthcare teams to develop and implement individualized care plans.
  • Perform phlebotomy and laboratory tests as needed.
  • Maintain accurate records in electronic health systems such as Epic or Cerner.
  • Educate patients and families on health management, medication administration, and pain management strategies.
  • Support case management efforts by coordinating with other healthcare professionals.
