Description Nexus Family Healing - Austin campus looking to add a Licensed Practical Nurse - Temporary Relief to join our youth residential program! Location/Pay/Schedule: Located at 1111 28th St NE, Austin, MN 55912 Starting Salary: $24 - $28 per hour, offer will reflect level of experience On-Call hours Position Summary: The LPN provides medical services, procedures and protocols that promote resident health and wellbeing. Primary responsibilities: Client Centered Medical Care Coordinates necessary follow-up appointments and/or referrals with interdisciplinary teams including InKind staff, psychiatrists and Mobile Response Coordinators and communicates medical concerns. Collaborates with other interdisciplinary teams regarding medical aspects of client admissions. Assists with all aspects of the client admissions, and nursing assessments with exception to medical exam. Collects and reviews clients’ medical data and ensures that medical records are current and accurate. Assesses pertinent client data and determines which areas need to be communicated with the physician. Monitors acute and chronic medical conditions of clients; provides treatment or consultation as needed. Gathers relevant information and assesses client status in the event of emergency situation. Provides appropriate care and/or treatment and directs others as necessary while collaborating to provide and facilitate a referral to higher level of care when appropriate. Treatment Team Collaboration Serves as a resource to staff and other interdisciplinary teams regarding client medical concerns. Collaborates with the treatment staff to develop individual client treatment plans. Promotes client health & well-being Performs centered health care services, in accordance with the Nexus policies, procedures and protocols. Completes all required trainings that are department role specific and facility -wide requirements. Maintains and stays apprised with trainings and certifications (online trainings, internal and external, in-person trainings, CPR certifications, nursing license, first aid and safety, medication administration, and infection control etc. Participates in internal training opportunities with community partners. Client Confidentiality Ensures client information remains confidential and compliant with Nexus policies involving client privacy, informed consent and confidentiality standards for clients and their families. Implement, monitor and coordinator the administration of client medications. Verify, count, and administer medication. Educates clients on medication side effects, adverse effects, and drug interactions. Monitors clients daily for side effects, adverse effects and or drug interactions. Ensures client prescriptions are filled, and the appropriate amount is maintained through each client’s stay within the resident milieu. Monitor medication logbooks for completion and accuracy. Requirements Required Education and Licensure: Graduate of an accredited school of Practical Nursing 2+ years’ experience in a clinic and/or hospital environment Valid driver’s license required.
