The Position: We're looking for passionate Licensed Practical Nurse who provides professional nursing care to patients following established policies and procedures of the agency. * Assumes overall responsibility for an assigned case load of home health patients, providing assessment, coordination of ancillary services and community services, and may provide direct nursing health to the patient in the patient's place of residence. * May oversee and direct Licensed Practical Nurses (LPNs) Home Health Aides (HHAs) in their day-to-day assignments. * Answers questions from patients, responsible parties and other personnel.
