In this role, you aren’t just a nurse; you are a teacher and a coordinator who builds lasting relationships with medical providers and families to ensure well-managed, proactive healthcare is received.   What YOU'LL Do: * Medical Monitoring: Monitor daily documentation for medical needs, identify health trends promptly, and perform health monitoring of documentation to ensure high scores in quality audits. * Care Coordination: Coordinate with pharmacies and doctors, complete medical appointments, and provide advocacy at critical care consultations. * Clinical Support: Provide hands-on care when needed, including wound care, ostomy care, catheter management, and monitoring critical care medical needs.  * Education & Instruction: Teach medication administration refresher courses, lead clinicals for CMAs and Medication Managers, and provide on-site instruction for staff regarding care plans and medical protocols. * Administration: Update and maintain care plans and medical protocols while utilizing technology (Therap) for accurate documentation and regulatory compliance.
