  POSITION OVERVIEW Responsible for ensuring the integrity and confidentiality of client medications. Monitor all client's medication regiment to include IM's. Work with healthcare providers in coordination of care for all clients. Assure all medications are in accordance with agency standards and various government agencies. Assure all client information in regard to medication is in our electronic health record system. Information contained in this job description is a summary of the assigned duties and not inclusive of all requirements that may be assigned. Additional duties may be assigned on an as needed basis. SPECIFIC RESPONSIBILITIES - Responsible for medication management for PRP, RRP, Deaf Services, TAY and ICM. - Tracking IM Due dates. - Tracking lab work due dates. - Med audits. - Review MAR's compare to practice fusion prior to Mar's going out at new month. - Review of schedule II meds (documentation med counts, etc.). - Pharmacy deliveries (deliveries are twice a day) - Training new staff on medication policy and procedure. - Coordinate hospital discharges - Track medication changes. - Updating practice fusion (HRIS) - Following up on hospital discharges and possible med changes - Communication with Dr. offices regarding medication - Communicating with pharmacy - Communicate with case managers and residential staff as needed - Maintain med storage protocols and chain of custody. - Assuring compliance with policies/procedures, JACO and COMAR/ PRINCIPAL ACCOUNT ABILITIES - Medication Management - Client safety regarding medication and coordination of care. EDUCATION, WORK EXPERIENCE AND JOB REQUIREMENTS Education: High School diploma with college coursework completed for accredited practical nursing certificate program. Licensed Practical Nurse license required in the State of Maryland. OJT: One to three months training required for effective performance Job Requirements: Ability to work with little or no supervision. Must have excellent organizational, and communication skills. Must be familiar with Windows and Microsoft Office applications. Knowledge of mental health diagnosis and disorders.
