Description POSITION SUMMARY: The purpose of this position is to provide general nursing care and leadership, working collaboratively with physicians and multidisciplinary team members. The L.P.N. will promote and restore patient’s health as they implement, delegate, evaluate, document and coordinate professional patient care through the use of the nursing process for patients of all ages, gender, cultures and background by providing physical and psycho-social support to patient, friends and families. The L.P.N. will plan for the provision of patient care by implementation of the plan of care in order to deliver efficient, effective and quality patient care with oversite of other personnel which includes but not limited to Certified Nursing Assistants. The L.P.N. must be able to demonstrate the knowledge and skills necessary to provide appropriate care and is expected to adhere to and abide by policies of the facility and the rules and regulations set forth by the Oklahoma Nursing Practice Act. PRINCIPAL JOB DUTIES AND RESPONSIBILITIES: Patient Duties: Direct and evaluate total nursing care. Uses good communication skills to interact with patients, families and team members. Assist Case Management by being a patient advocate, resource and contact person for patients. Provides basic education information to patients and family, focusing on self-care appropriate to their age and identified learning considerations and needs. Performs assessment data collection and documents subjective data (chief complaint, allergies, etc.) and objective data (vital signs, weight, etc.). Reviews basic health history information for patients focusing on environmental, self-care and discharge planning. Administers medications and intravenous solutions skillfully and correctly using the 7 patient rights, accurate patient identification and performs lab/diagnostic testing as ordered by the provider and reports all critical lab values immediately. Demonstrates an ability to remain calm, perform effectively and professionally during peak periods of activity and emergency situations. Assists provider with examination and procedures per request. Other duties as required. Utilizes universal precautions in obtaining specimens (venipuncture, cultures, etc.) and in delivery of patient care. Team Duties: Evaluates the effectiveness of the care given by the interdisciplinary care team and identifies problems and guides team members to their solution. Assigns care to team members in accordance with patient needs, staff capabilities and qualifications. Participates in training, orientation, competency assessment and education of new employees. Communicates clearly, concisely and accurately, creating a working climate that promotes growth and job satisfaction of personnel. Communicates with The R.N. effectively. Uses effective coaching, positive reinforcement and other methods to enhance team performance. Serves as a resource person to other staff members. Arrives on time, works scheduled shifts and badges in and out appropriately. Other duties as required. Administrative Duties: Acts in a professional manner, reflecting the mission of Cleveland Area Hospital and demonstrates compliance with all policy, procedures and state/federal regulations. Demonstrates ongoing service excellence skills. Maintains economical and efficient nursing care in a clean and safe environment. Participates in performance improvement activities as required. Demonstrates ongoing understanding and supports corporate strategic initiatives. Participates in meeting, education and demonstrates genuine desire to develop strategies for improved operation for a higher level of patient care. Other duties as required. EQUIPMENT: Computer and Printer Gloves Shoe Covers Copy Machine Gowns Fax Machine Respirator Telephone Goggles/Face Shield Requirements EDUCATIONAL/SKILLS REQUIRED: Current L.P.N. license recognized and in good standing with the Oklahoma Board of Nursing BLS ACLS – Current or within 6 months of hire PALS – Current or within 6 months of hire Able to make decisions under pressure. Able to handle multiple priorities EXPERIENCE: One year required RESPONSIBILITY FOR CONFIDENTIAL INFORMATION: This position could have access to patient confidential records and must comply with HIPAA regulations as well as Cleveland Area Hospital Policies.
