Overview To provide practical nursing services under the supervision of a Registered Nurse. Responsibilities Accepts assignments of clients from the R.N. Case ManagerProvides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plansa. Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager. b. Observes signs and symptoms and reports them to the R.N. Case Manager. c. Notes changes in physical or emotional status of clients. d. Promotes client and caregiver independence in self-care Utilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of dataFollows Agency policies regarding universal precautions, and safety guidelinesFulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services. Qualifications Education and Experience: Holds a current Licensed Practical Nurse license in VirginiaHas a minimum of one year of nursing experienceHas current CPR certificationHas excellent communication, organizational, and human relations skillsIs able to function with minimal supervisionIs able to manage multiple projects/changing prioritiesIs able to employ critical thinking skills
