Position Summary Responsible for performing a variety of duties to provide quality nursing care to program participants and to coordinate nursing care for program participants, implementing specific procedures and programs. Requirements MINIMUM QUALIFICATIONS 1.Graduate of an approved school of nursing; registered nurse or LPN with a current license to practice in the Commonwealth of Pennsylvania. 2.Experience with or knowledge of acceptable assessment instruments used in evaluation of human growth and development. 3.Experience working with individuals with intellectual and physical disabilities preferred. 4.Valid PA driver’s license. DUTIES AND RESPONSIBILITIES 1. Provide a supportive, caring, safe environment by attending to individual needs, respecting differences, and supporting growth and independence. 2. Training and supervision of medical plan of care. 3. Involvement in referral process (intake, assessment, placement, transfer) and supervision of referrals as requested. 4. Preparation of required or requested reports. 5. Attend meetings as required and act as a member of the Interdisciplinary Team. 6. Evaluation and monitoring of medical needs of program participants to ensure that appropriate health services are provided in collaboration with program staff. 7. Coordination of scheduling and attendance at individuals’ medical appointments as needed. 8. Complete assessments of program participants and analyze information to develop appropriate interventions to prevent decline; record signs of change in condition; notify appropriate staff per facility protocol to evaluate weight loss, decline in ambulation, development of skin breakdown, etc.; observe conditions which indicate possible need for restorative nursing programs and then notify appropriate staff per facility protocol of the need for evaluation; inform the physician of changes in assessment when appropriate. 9. Administer medications and treatments as needed to program participants; assist with placing orders for medication from the pharmacy as needed. 10. Assess program participants upon admission or re-admission to develop interim care plan and further assess in depth to contribute to the complete care plan. Attend and participate at ISP and SIS assessment meetings as needed. 11. Assess the program participant’s condition routinely and as needed based on health status; decide on the type of medical attention; notify family when program participant is transferred or admitted to the hospital; notify the physician when a change in condition occurs. 12. Assist with maintaining program participant clinical files; keep charts updated; document appropriately. 13. Assist with HRST rating as requested and complete nurse reviews in a timely manner. 14. Review controlled substances count sheets; inventory and review medication expiration dates and availability as well as confirming pharmacy label matches CaraSolva regularly; notify program supervisor of discrepancies. 15. Communicate status of program participants according to established procedures; communicate with physicians, clarify, and implement physicians’ orders. 16. Provide on-the-job training, evaluate performance, and provide direction. 17. Regular attendance at work. 18. Other duties as assigned. PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT • Physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of the Healthcare Services Specialist’s job. Example: avoiding trips and falls, back care, and observing fire regulations. • May need to sit or stand. May be required to walk on a level surface periodically throughout the day. May be required to reach above the shoulder, below the waist or lift documents or materials throughout the workday. Proper lifting techniques are required. The Healthcare Services Specialist is required to lift and/or move up to 35 pounds. May be required to walk up and down stairs periodically throughout the day.
