Position Summary: The Licensed Practical Nurse administers nursing care to designated residents under the direction of a registered nurse. Coordinates and supports the activities of the certified nurse assistants and actively participates in the direct care of the resident, by following the specific approaches as stated in the plan of care. Updates and communicate ISP plan of care that identifies the health problems/needs of the resident and the goals to be accomplished for each health problem/need identified. Administer and record treatments/medications Document resident care, condition, and response to care. Clarify and implement physician orders. Determines need to transfer resident to hospital and take necessary steps to ensure smooth transition. Coordinate re-admissions from hospital. Communicate the resident’s condition to physician, Director, and responsible party promptly and take appropriate action. Count medications, administer treatments/medications according to physician’s orders and accurately record treatments/medications administered. Provide direct resident care and provide clear instruction to aides. Other responsibilities may be assigned. Key Requirements Other: Participation in scheduling, being flexible, and willing to cover shifts as necessary. Ability to provide direct resident care and provide clear instruction to care partners. Ability to listen to resident, family, or physician concerns and implement appropriate action.
