Description Job Summary The Licensed Practical Nurse (LPN) provides direct nursing care to residents under the supervision of a Registered Nurse (RN) or physician. The LPN ensures that residents receive quality, compassionate care in accordance with physician orders, care plans, facility policies, and state and federal regulations. Key Responsibilities Resident Care Provide direct nursing care to residents, including monitoring vital signs, administering medications and treatments, and assisting with daily care needs. Observe and report changes in resident condition to the RN or physician. Perform wound care, catheter care, and other nursing procedures as directed. Assist with admissions, discharges, and transfers. Document nursing care accurately and timely. Clinical Support Follow individualized care plans and contribute to care plan updates. Assist with restorative and rehabilitative nursing services. Ensure proper infection control and safety practices. Team Collaboration Work collaboratively with RNs, CNAs, and interdisciplinary team members. Provide guidance and support to CNAs as assigned. Participate in shift reports and care conferences as needed. Compliance & Resident Rights Ensure compliance with facility policies and regulatory requirements. Maintain resident confidentiality, dignity, and rights at all times. Report incidents, accidents, and concerns appropriately. Qualifications Current Licensed Practical Nurse (LPN) license in the state of employment. Must have a current CPR/BLS Skilled nursing or long-term care experience preferred. Knowledge of geriatric care and basic nursing practices. Strong communication and organizational skills. Compassionate, dependable, and team-oriented. Requirements Physical & Work Requirements Ability to lift, transfer, and assist residents (up to 50+ lbs with assistance). Ability to stand, walk, bend, and move throughout the shift. Exposure to infectious diseases and bodily fluids. Must be able to work weekends, holidays, and varied shifts as needed. Ability to manage the physical and emotional demands of resident care.
