The LPN Supervisor oversees day-to-day staffing and clinical workflow on the floor to support safe, person-centered resident care in accordance with Maryland COMAR 10.07.14 and facility policy. The role leads daily assignments, ensures documentation requirements (including weekly nursing notes) are met, conducts provider rounds, fields staff calls, and engages the clinical on-call service. The LPN Supervisor nurtures teamwork, coaches front-line staff, and helps maintain survey readiness. Essential Functions: 1) Clinical Operations & Documentation Lead the nursing huddle and daily clinical workflow; ensure care delivery aligns with current service plans and provider orders. Ensure care/nursing notes are entered at least weekly for each resident and at other required triggers (admission, significant change, transfers, post-appointment, provider in-house visits, nonroutine leave, discharge). Complete LPN-level assessments/observations and progress notes within license scope; immediately escalate significant changes to the RN/DN and provider; assist with implementing updated orders. Participate in and document interdisciplinary/provider rounds (e.g., NP/MD, therapy, pharmacy) and track recommendations to closure. Support accurate, timely incident reporting and follow-up per policy. 2) Medication Administration & Safety Administer medications and treatments within LPN scope; verify rights of medication and accurate documentation. Support safe storage, counts, reconciliation, and disposal processes for medications (including controlled substances) consistent with policy and COMAR. When CMTs or unlicensed staff administer medications under RN delegation, ensure they follow policy/procedure and promptly escalate concerns to the DN/RN. 3) Staffing, Scheduling & Supervision Build and maintain monthly/weekly/daily schedules for care staff; ensure on-site 24-hour coverage and relief personnel per plan. Adjust staffing against acuity/needs (e.g., awake overnight staffing when indicated by assessment) and notify DN of variances or risks. Directly supervise non-licensed staff during assigned shifts; provide assignments, real-time coaching, and performance feedback; initiate corrective action per policy. 4) Education, Competency & Compliance Conduct and document initial/annual trainings and in-services as assigned (CPR coverage on shifts, infection prevention, fire/life safety, emergency plans, food safety, residents’ rights, cognitive/behavioral health content as applicable). Validate skill competencies of non-licensed staff for personal care tasks as permitted by policy (with DN sign-off when required); ensure orientation completion before independent work. Assist with quality assurance activities, audits (eMAR, MAR, care notes, weights, pharmacy review follow-up), and mock surveys. 5) Communication & Service Recovery Field staff calls and triage/escalate clinical questions to the on-call RN/DN; document guidance and outcomes. Communicate with residents/families on routine matters and escalate complex issues to the DN or Executive Director. Facilitate timely provider communication for medication changes, condition updates, and lab/diagnostic coordination. Driving Requirements: Must be able drive and use personal vehicle. Milage reimbursement offered. Management/Supervisory: Oversee staffing of Nursing department. Medication Technicians, CNAs, CMAs, resident care providers and/or LPNs. Requires routine on-site presence, with 24/7 availability for clinical consultation and crisis escalation. Requirements Professional Qualification: Positive; Able to manage multiple high-level tasks such as scheduling staff; scheduling care conferences and maintaining communication with staff and families in a professional, positive manner. Must maintain CEU and other state requirements for the position, as applicable. Level of Education High School Diploma/GED. Must be at least 18 years of age. Technical or Professional License(s)/Certification(s) Active MD LPN Current CPR. Level of Experience : 3+ years direct care in senior care/AL; prior charge/supervisory experience preferred. General Knowledge/Skills/Abilities: Must be able to speak and understand English. Display positive attitude toward the residents, visitors, staff and families. Ability to communicate with residents and remain calm under stress. Ability to understand and apply training and in-service education.
