What You’ll Do The Licensed Practical Nurse (LPN) works collaboratively with the care team to provide ongoing support and communication to chronic kidney disease (CKD) and End-Stage Renal Disease (ESRD) patients. The LPN acts as a single point-of-contact to coordinate resources along the care delivery spectrum, identify gaps, and provide proactive follow-up. The LPN is responsible for making sure the patient’s care at various locations is connected and there are no gaps in care or communication. This role will report to the Sr. Manager, Market Operations. The Day to Day Performs outbound calls to patients to understand their clinical needs and connect them with appropriate resources, following up with patients to ensure their needs are met and scheduling future check-ins. Performs outbound calls to providers to make appointments for patients or follow up on care. Responds to inbound calls from patients, providers, and other resources. Monitors patient hospitalizations and follows up as necessary with care team members and outside resources to confirm Strive gathers all relevant patient information. Provides patients with education materials and sends communications to primary care physicians, nephrologists, and specialists for new enrollments/appointments. Provides health coaching to patients on disease and symptom management. Partners closely with the Strive Nurse Practitioner to manage all pieces of care related to resources, appointments, care transitions, and care gaps. Provides in-person patient care which may include standing, sitting, walking, pushing, pulling, and lifting. Minimum Qualifications 5+ years combined of related education, experience, or certification. Active LPN (Licensed Practical Nurse) Licensure in state of practice. Efficient and reliable transportation, including an active driver’s license, allowing for the ability to travel across an assigned region to meet patient needs.
