Responsibilities Provides compassionate, hands-on nursing care for patients with physical disabilities and chronic illnesses using the nursing process to achieve quality outcomes. Collaborates closely with RNs, interdisciplinary rehabilitation teams, patients, and families to coordinate individualized plans of care and support recovery goals. Assists with patient and family education, discharge planning, and reinforcement of teaching to promote independence, wellness, and self-management. Demonstrates strong clinical judgment, problem-solving, delegation, and time-management skills within the LPN scope of practice. Supervises and provides direction to unlicensed assistive personnel while reinforcing care provided by other healthcare team members. Contributes to the assessment, implementation, evaluation, and modification of patient care plans to address physical, psychological, sociocultural, spiritual, and educational needs. Communicates effectively with patients, families, and healthcare professionals to build rapport, support teamwork, and ensure accurate legal documentation and reimbursement requirements. Participates actively in interdisciplinary team meetings and advocates for patient rights, recovery opportunities, and cost-effective care solutions. Promotes a safe environment by prioritizing patient and workplace safety, preventing errors and infections, and reporting concerns promptly. Maintains compliance with federal, state, and accrediting agency standards while supporting excellence in rehabilitation and patient-centered care Qualifications Requirements: Diploma from an LPN program. Current licensure in good standing to practice as a Registered Nurse/Practical in primary state. Must apply within 90 days for licensure in secondary state. Must possess and maintain current Basic Life Support (BLS) certification.
