UPMC Northwest Hospital Rehabilitation Unit Licensed Practical Nurse Status: Flex Full Time (28-40 hours/week) Shift: Variable Responsibilities: Deliver nursing care using focused assessments and established care plans in accordance with UPMC policies, LPN Practice Guidelines, and the State Practice Act Communicate patient condition, progress, and changes to the licensed nurse and healthcare team; seek guidance when care needs exceed scope, skill, or patient condition changes Administer treatments and perform procedures, including IV therapy, within LPN scope of practice Document patient care accurately and thoroughly to support continuity of care Provide education to patients and families to support care, discharge, and transitions Foster a compassionate, patient-focused environment through Relationship-Based Care Participate in quality, safety, and patient experience initiatives Support orientation and development of new LPNs and care team members Collaborate effectively with patients, families, and interdisciplinary team members to achieve desired outcomes Identify and communicate safety concerns and opportunities for clinical or process improvement Perform duties in accordance with system-wide competencies and other duties as assigned IV therapy curriculum required within 4 months of hire/transfer Graduates from 2012 to present: IV therapy included in PA State Board–approved programs Graduates prior to 2012: official proof of completion of an approved IV therapy program required Ability to practice at the top of LPN license in Med/Surg, Telemetry, or Emergency Department settings Ability to collaborate with the healthcare team, patients, and families to achieve optimal outcomes Strong interpersonal skills to establish positive working relationships across all levels of the organization Physical ability to meet job demands, including standing, walking, lifting, and positioning patients Computer proficiency, including use of electronic health records Licensure, Certifications, and Clearances Current Licensed Practical Nurse (LPN) license in the state of practice or a multistate license through the Nursing Licensure Compact (NLC)* Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification Act 34 Clearance OAPSA Clearance
