As the Licensed Social Worker (LSW), you are the bridge between patients, families, and the resources they need. Impact: Your work will bring comfort, dignity, and peace to patients and families Collaboration: Join a team of experienced professionals that value your expertise Growth: Access ongoing training, certifications, and advancement opportunities to grow your career Key Responsibilities Comprehensive Assessments: Evaluate psychosocial status of patients and families related to terminal illness and living environment Care Planning: Develop and implement interventions based on thorough evaluations Counseling & Support: Guide patients and caregivers through stress, coping challenges, and end of life issues Documentation: Maintain accurate, timely clinical records for all referred patients Resource Navigation: Provide referrals and serve as a liaison with community agencies Team Collaboration: Participate in interdisciplinary care planning and regular team meetings Advocacy: Help physicians and team members understand social and emotional factors impacting care Quality Improvement: Actively contribute to performance improvement initiatives Flexibility: Perform additional duties as delegated by the Clinical Supervisor Requirements Bachelor’s degree in Social Work from a CSWE-accredited program Minimum 1 year in healthcare, hospice experience preferred Deep understanding of hospice philosophy and end-of-life care Current CPR certification required Valid driver’s license, auto insurance, and reliable vehicle for community visits The employer for this position is stated in the job posting.
