Licensed Social Worker (MSW) – Care Management Location: Washington Court House, Ohio Department: Care Management / Social Services Employment Type: Full-Time Shift: Day shift Position Summary The Licensed Social Worker (MSW) provides direct patient care and care management services across the continuum of care. This role collaborates with multidisciplinary healthcare teams to assess patient needs, coordinate care, support discharge planning, and promote safe transitions of care. The Social Worker serves as a patient advocate while ensuring compliance with state and federal regulations, including CMS Conditions of Participation. This position supports patients of all ages through holistic assessment, counseling, crisis intervention, referral coordination, and education to achieve optimal health outcomes and appropriate utilization of healthcare resources.
