JOB SUMMARY

We are seeking licensed LMSWs, LCSWs, or LMHCs to join our team at adult homes serving residents aged 65+. This role involves primarily one-on-one counseling sessions and some group therapy, addressing mild to moderate depression, anxiety, and alcohol or substance use issues. You will work in an integrated care model, alongside medical doctors and a collaborative team of social workers.

QUALIFICATIONS

• Active LMSW, LCSW, or LMHC license in New York State (required).
• Experience in behavioral health, geriatric care, or substance use counseling (preferred).
• Strong counseling and group facilitation skills.
• Excellent communication, teamwork, and documentation skills.
• Bilingual (English/Spanish) a plus.

• Provide individual counseling sessions to older adults (65+) for depression, anxiety, adjustment, grief, and substance use concerns.
• Facilitate group sessions on coping skills, relapse prevention, and emotional wellness.
• Conduct psychosocial assessments and develop individualized treatment plans.
• Coordinate care with physicians, psychiatrists, and nurses to ensure continuity.
• Offer crisis intervention, resource navigation, and advocacy support.
• Monitor client progress, update care plans, and maintain accurate documentation.
