Position Summary The Social Services Manager plays a key role in advocating for residents and ensuring their best interests are always at the forefront. You’ll support residents through life changes, help them access essential resources, and work closely with families and caregivers to provide thoughtful, individualized care. Note: All Cascadia Healthcare employees must complete and pass their state’s required background check prior to having contact with residents or patients. Essential Functions Assess residents’ psychosocial status and collaborate with nursing staff on care needs. Conduct social evaluations and develop personalized, resident-centered care plans. Maintain accurate clinical documentation for all residents referred to social services. Provide guidance and coordinate referrals for residents, families, and caregivers regarding community resources and support services. Serve as a liaison between residents, families, caregivers, and community agencies. Build strong, collaborative relationships with facility staff to support high-quality care. Participate in interdisciplinary care plan development and case conferences. Help team members understand social and emotional factors affecting resident health. Develop behavior monitoring plans and interventions aimed at improving well-being and minimizing unnecessary medication use. Address resident behaviors in a proactive and supportive manner to promote safety. Lead and support discharge planning to ensure smooth transitions and continued support post-discharge. Coordinate medication review processes to help reach the lowest effective dosages when appropriate. Knowledge, Skills & Abilities Strong verbal and written communication skills Excellent organization and time-management abilities Ability to collaborate effectively across disciplines Compassion, empathy, and a dedication to resident advocacy Familiarity with community resources and support networks Requirements Education Bachelor’s or Master’s degree in social work. Licenses/Certification LCSW, LSW, or MSW license required. Must have a valid driver’s license. Experience Experience in post-acute/ skilled nursing preferred.
