YoloCares (formerly Yolo Hospice) is Joint Commission accredited and the only organization in the Sacramento region to be accepted into the esteemed National Partnership for Hospice Innovation. Essential Job Functions & Responsibilities: Providing direct patient care under the supervision of the Hospice Clinical Manager. Makes home visits as necessary. Works cooperatively with patients, families, nursing staff, physicians and ancillary service providers. Conducts patient triage from patients/families. Requirements Job Qualifications: A current license in the State of California as a Licensed Vocational Nurse. Ability to communicate effectively verbally and in writing in English is required. Spanish language skills are also encouraged. One (1) year of hospice experience desired; two (2) years SNF/home care experience. Knowledgeable about Hospice Medicare Conditions of Participation and California Hospice Licensing Standards.
