Job Details Job Location: Houston, TX 77063 Position Type: Per Diem Salary Range: $30.00 - $34.00 Hourly Travel Percentage: Road Warrior Constellation Health Services is looking for a compassionate and skilled Licensed Vocational Nurse (LVN) to join our team! For The GREATER HOUSTON AREA (approx. 16-24 hrs. per week must have availability Monday thru Friday during the day!) You will play a crucial role in providing end-of-life care and hospice services to patients in their homes and communities. Under the guidance of an RN, you'll assist in planning patient and family care, follow physicians' care plans, administer treatments, and collaborate with our dedicated team to ensure positive patient outcomes. Responsibilities: Deliver nursing care in accordance with the State Nurse Practice Act, with a focus on end-of-life care. Implement current nursing practices based on comprehensive assessments and care plans. Maintain accurate and timely documentation of patient services. Provide patient and family/caregiver education and support. Recognize and address changes in a patient's condition. Collaborate with interdisciplinary teams, such as RNs, hospice aides, social workers, and more. Participate in Hospice IDT meetings to ensure coordinated care. Use and maintain equipment and supplies effectively. Manage pain and symptoms, evaluating patient responses to treatments. Qualifications Qualifications: Graduate of an accredited practical nurse program. LPN/LVN licensed in the state of employment. Preferably, experience in hospice, palliative or community health home care or medical/surgical settings. Background in clinical auditing and chart reviews is a plus. Wound care experience is preferred. Understanding of hospice philosophy and the needs of the terminally ill. Basic knowledge of iOs operating system
