We are a 144-bed Skilled Nursing Facility with rehabilitation services located in the heart of Santa Ana. We are currently looking for a Per-Diem LVN who is excited to be a part of a culture that celebrates a supportive environment!  Shifts Available:  * Per-Diem Day Shift (AM) Pay Range: * $35-45/hour Depending on Experience  Position Overview:  The LVN is responsible for assisting RN’s in conducting the day-to-day functions in accordance with each resident’s care plan.    * Monitor residents for changes in condition and promptly report findings to RNs or physicians * Administer medications and treatments per physician orders and scope of practice * Perform wound care, catheter care, and other clinical interventions as needed * Collaborate with CNAs and interdisciplinary team members to ensure holistic care * Maintain accurate and timely documentation in electronic health records (EHR) * Support infection control, safety protocols, and emergency procedures * Provide emotional support to residents and families, promoting resident dignity and rights * Participate in shift reports, care planning, and quality assurance initiatives Qualifications: * Active LVN License in CA  * CPR/BLS * Strong clinical judgement, teamwork, and communication skills We provide equal employment opportunities (EEO) to all employees and applicants for employment without regard to race, color, religion, creed, sex (including pregnancy, childbirth, or related condition), age, national origin or ancestry, citizenship, disability, marital status, sexual orientation, gender identity or expression (including transgender status), genetic predisposition or carrier status, military or veteran status, familial status, status as a victim of domestic violence, or any other status protected by law
Responsibilities
