Description JOB SUMMARY: Provides nursing care to assigned patients under the supervision of a licensed professional to meet regulatory standards in the Nursing Home. Assists the professional nursing staff in providing maximum patient care, assessment, planning, implementation and evaluation of each assigned patient. Prepares and leads a nursing team and administers treatments and medications within limitations and under guidance. Provides direct and indirect patient care as assigned. OSHA Bloodborne Pathogens Class I. ESSENTIAL DUTIES AND RESPONSIBILITIES Provides a variety of vocational nursing functions as a healthcare team member in the Nursing Home. Obtains a complete report of resident needs, attends conferences and plans work activities according to assignment. Admits new patients and participates in the formulation of their care plan and assists in dismissal of patients according to procedure. Performs or assists patients in personal care measures, such as baths, showers, oral hygiene, meal service, AM and PM care and comfort measures, positioning and body alignment, making beds. Performs clinical nursing tasks as indicated in assignment, to include catheterization and routine care to indwelling cathethers. Applies various medicated dressing or compresses, gives enemas, Assists the RN with lavage and gavage. Assists physician with physical examinations and patient rounds. Prepares patient for diagnostic procedures, providing instruction to family and significant others. Assists patient with rehabilitative measures per physician orders. Administers medications according to policy. Recognizes actions, indications, adverse reactions, dosage and route of administration on all drugs and accurately verifies medication administration record. Audits narcotic count with previous or oncoming medication nurse, witnesses wastage of narcotics according to procedure. Obtains stock supplies and individual medication. Transcribes orders to medication and treatment cards, per procedure. Records patient treatments in medical record system and provides information to RN and physician. Documents teaching completed and patient and family response. Provides care appropriate for the geriatric patients served based on demonstrated knowledge of the principles of growth and development over the life span. Assists in orientation of new patients to their environment. Assess and interprets data to identify patient requirements related to their age specific needs and provides care needed as stipulated in departmental policy. Provides service direct to patients while maintaining a positive patient relations atmosphere. Acts as liaison between patient, physician, family member, significant other and other health disciplines. Requirements Education – Graduate of an accredited School of Vocational Nursing Job Related skills – Demonstrated effective oral and written communication skills License, Registry or Certification – Licensed Vocational Nurse currently licensed in the State of Texas. Current BLS certification Experience – Prior Work Experience - One year as a licensed Vocational Nurse preferred. Technical Training – Computer skills required. Physical and Mental Requirements - Position requires extended periods of walking and standing. Repeated bending, stooping and reaching, lifting up to 50 pounds. Transferring and lifting or supporting patients who may experience sudden weakness or inability to stand. The ability to comprehend and follow written or verbal instructions and directions and to instruct employees. OTHER SIGNIFICANT REQUIREMENTS Follows aseptic procedures and procedures for using precautions when involved in direct patient contact or handling potential infectious materials. Interacts with patients, family members and Medical Staff members on a continuous basis. Maintains HIPAA required and all other confidentiality while working with confidential matters on a daily basis. Follows MCHD Policies and Procedures and safety guidelines. Performs other duties as assigned by the department management staff. Performs essential job functions with or without reasonable accommodations.
