The role of the Life Coach is to support the planning, preparation and development of competencies for youth from age 15 to 25 who are or already transitioned out of foster care. They are youth and young adults who have experienced trauma and have behavioral health issues. We also serve youth in protective supervision and/or relative caregiver placement but are still in need of assistance to achieve self-sufficiency. Provide direct services to program participants including but not limited to: participate as a member of the wraparound team, provide youth development activities, assessments, service planning, linking & coordinating community resources and all program components (i.e. field trips, community projects, employment activities, health education, etc.) Conduct Independent Living Life Skills Assessments for eligible program youth Conduct Aftercare/Transitional Services Needs Assessments Develop an Individualized Life Plan and life skills education plan for each youth Arrange and provide groups, field trips, community projects, special events and other opportunities Track grant required outcomes and report on them every 6 months. Provide targeted Aftercare services for over 18-year olds who are receiving follow-up services Maintain program documentation on assessments, training and services specific to the youth Maintain files, youth applications, assessments and life skills education plans Conduct ILP Reviews/ Staffings in the absence of the Director of Independent Living Provide like skills coaching and support to youth in the program, as well as provide life skill activities (i.e. setting up a back account, finding housing, applying for a job, etc.) We invest in you! Qualifications MINIMUM QUALIFICATIONS: Minimum of a Bachelor’s degree in social/human services; Master’s degree is a plus; Minimum 1 year experience in child welfare and human services; knowledge & experience with ESE youth and Independent Living preferred. Ability to work flexible hours. Program’s general operation hours are Monday through Friday from 8am to 8pm with weekends, as needed. Position is a 40-hour work week with hours varying on client needs and with weekly productivity expectations.
