Position: Life Coach XI Salary: $22.50 per hour Responsibilities: The Life Coach XI is responsible to provide direct training services and professional supervision over the daily programs provided by the direct service personnel. Job Duties: ~ Will provide person-centered case management services. ~ Support individuals acquiring skills in areas of their person-centered plans. ~ Conduct staff meetings, prepare reports for and participate in case conference. ~ Assure implementation of appropriate community activities in person-centered plans. ~ To assure annual program and agency goals are met. ~ To maintain monthly/regular contact with correspondents to coordinate services. Qualifications: ~ High School diploma or equivalent and three years working in the human services field, with at least one of those years working with people with disabilities or Bachelors Degree in Human Services, with at least one year working with people with disabilities.
