Description JOB SUMMARY An individual who assists the Life Enrichment Director in assessing, planning, implementing and evaluating activity programs for residents within care units. Directly responsible to the Life Enrichment Director ESSENTIAL JOB FUNCTIONS: Maintains and actively promotes effective communication with residents/patients/clients and family members / personal representatives. Responsible for maintaining a positive image of the entity in the community keeping in alignment with our mission, vision, and values. Will read and adhere to resident rights agreement and report any violations of same immediately Will maintain a calm composed attitude when dealing with residents, families, or staff who may exhibit challenging behaviors. Will work cooperatively with all disciplines including volunteers. Will provide life enrichment of therapeutic value, following the accessing, planning, implementing and evaluating process to include documentation of enrichment. Will promote optimal resident participation in activities and support transportation to/from activities. Will complete MDS documentation, care plans and assessments as assigned on a timely basis, maintaining confidentiality. Leads resident council meetings, maintaining/sharing minutes through appropriate channels and resources. Will report any unusual resident’s behavior and health concerns to nursing staff. Maintains order and cleanliness in activity areas. Treats residents, family and staff with dignity and respect. Researches and supports supply fulfillment for life enrichment activities. Other duties as assigned Requirements QUALIFICATIONS 1. Commitment to quality programs and services for the residents/patients/clients. 2. Excellent communications and human relation skills. 3. Ability to relate well to the residents/patients/clients, family members / personal representatives. 4. Ability to maintain and protect the confidentiality of information. 5. Ability to exercise independent judgment and make sound decisions and exhibit flexibility when needed. 6. Ability to work independently with flexibility in scheduling activities. 7. Ability to lead large and small group activities and get involved, i.e. wear costumes, use props, etc. 8. Must have a valid driver’s license and a good driving record. EXPERIENCE AND TRAINING 1. High school graduate or equivalent 2. Ability to utilize common word processing and document activities in medical record. 3. One year experience with residents or individuals w/special needs.
