Description

You will have the opportunity to:

  • Support residents' goals of care in partnership with each resident and/or their representative
  • Assist with the assessment, planning, delivery and evaluation of personalised leisure and recreation programs in collaboration with residents, the Recreation and Engagement Coordinator and the broader care team
  • Facilitate a variety of individual and group activities that promote social, emotional and physical wellbeing, helping residents feel connected, heard and valued
  • Provide appropriate and stimulating programs for residents living with dementia and cognitive impairment
  • Maintain accurate documentation of assessments, care plans and participation records in line with organisational and aged care requirements
  • Apply a person-centred approach that recognises and promotes each individual's unique goals and preferences
  • Promote an inclusive and vibrant environment that encourages participation and enhances quality of life
  • Contribute feedback to improve programs and participate in risk management and continuous quality improvement activities
  • Build strong and collaborative relationships with your team to deliver positive outcomes for residents

About you

  • Certificate IV in Leisure and Health (or equivalent) or Certificate III in Individual Support with demonstrated experience in the leisure and health field or relevant experience in leisure and health service delivery.
  • Strong interpersonal and communication skills, with the ability to engage respectfully
  • Proficient written and verbal communication skills, including English proficiency
  • Competent computer skills, including basic documentation and use of digital tools
  • Strong organisational and time management skills, with the ability to work independently and as part of a team
  • A sound understanding of aged care and th
