You will be responsible for maintaining accurate documentation of activities including progress note entries and evaluation of the effectiveness of care services delivered. You will work across two locations: Monday at Prague House, Tuesday-Friday at Berengarra Classifications for this position will range from IN31 - IN33 ($55 510- $64 464 per annum) plus an additional overaward. Prague House is a purpose built 45-bed facility that caters to a resident group with a background of homelessness or significant risk of homelessness, often accompanied by some of the following - a history of poly substance misuse, acquired brain injury, complex mental health diagnoses and disabling social isolation.
