JOB DESCRIPTION SUMMARY

The Care Coordinator will be responsible developing a long-term relationship with our patients to effectively enroll them in the Link program or refer them to a participating provider. This role will serve as a communication liaison for the Territory Manager, Physician Office and Patient. The expectation is that they will be accountable for ensuring the patient’s enrollment is successful and all the supporting documentation is completed in a timely manner to allow the patient to receive supplies.

MINIMUM REQUIREMENTS

  • Maintain high attention to detail throughout daily tasks and projects including accurate notation and details of each customer account to ensure proper and timely follow up.
  • Assist other departments and team members when required or requested to help meet business needs.
  • Seek continual education opportunities to remain up to date with products and processes
  • High School Diploma or GED

PREFERRED REQUIREMENTS

  • Ability to solve practical problems and deal with a variety of variables.
  • 1+ years medical industry or call center/customer service experience

Responsibilities
  • Demonstrate comprehensive expertise in Home Care Urology product lines and ancillary products, including features, benefits, usage, and maintenance. Conduct initial intake and follow-up calls with patients to address concerns and provide solutions related to urological medical supplies and equipment.
  • Maintain detailed patient interaction records in Salesforce, coordinate with administrative support teams to ensure accurate documentation, and facilitate communication between patients, Territory Managers, and physician offices for document revisions through multiple channels including the Salesforce system, emails, and phone calls.
  • Meet or exceed departmental call volume requirements for both inbound and outbound calls, while adhering to Medicare guidelines, HIPAA regulations, ACW standards, and minimum call volume expectations established by Sales Operations management.
  • Maintain exceptional levels of customer service and professionalism during all interactions with patients, Territory Managers, physician offices, and fellow associates, contributing to a positive work atmosphere that reflects BD Home Care’s philosophy of professional excellence, teamwork, and integrity.
  • Work effectively with administrative support teams and Territory Managers to ensure seamless patient care, accurate documentation, and efficient resolution of issues, serving as a critical communication link between patients and the broader healthcare team.
