Hours: Monday – Friday, 8:30am to 4:30pm (occasional evenings and weekends)
Reports to: Prevention and Care Program Manager
For 35 years, the Indigenous Peoples Task Force has been at the forefront of closing the health equity gap for Native American and other marginalized communities impacted by HIV/AIDS across the Twin Cities and greater Minnesota. Based on Indigenous values and ways of knowing, our programs have grown to include services that promote integral role of and return to the centrality of first medicines, the use of cultural arts to support youth development, and programs that foster food security in ways that reconnect Indigenous and other peoples to the land and waters.

JOB SUMMARY

The Linkage to Care Specialist works collaboratively to help the Prevention and Care team identify, provide testing, and care resource referrals and actively connects newly and previously diagnosed persons, not-in-care who are living with HIV. Through partnerships with internal teams and other stakeholders, the Linkage-in-Care Specialist helps new and recently diagnosed People Living with HIV (PLWH) navigate HIV care and treatment systems and promotes medical adherence.

MINIMUM QUALIFICATIONS

A high school diploma and knowledge of HIV and STI modes of transmission, lifecycles, risk factors, harm reduction strategies and best practices; OR 2-5 years of direct social service experience.

QUALIFICATIONS

  • High School diploma
  • 1+ years of demonstrable understanding of HIV: its modes of transmission, lifecycle, risk factors, risk reduction strategies, and the impact of HIV on Indigenous communities
  • Working knowledge of National and Minnesota HIV and STI strategies
  • Minnesota certification to conduct HIV and other STI counseling, testing, and referral sessions
  • Experience providing services through culturally specific frameworks for Indigenous Communities
  • Working knowledge of HIV and other STI test technology
  • Knowledge of the intersection of substance use disorder, infectious disease risks, and treatment adherence
  • Knowledge of and experiencing in diverse communities and LGBTQ+
  • Strong interpersonal, communication, and collaborative skills
  • Understanding of confidentiality
  • Must possess a valid driver’s license
  • Ability to pass a criminal background check

PREFERRED QUALIFICATIONS

  • Undergraduate degree in any of the following: Biology, Nursing, Public Health, Social Work, Sociology, Psychology, Communication, or other relevant fields.
  • 5+ years of experience providing HIV and other STI counseling, testing, and referral services
  • Understanding of HIV and other STI surveillance and epidemiological trends in Minnesota and nationally
