The Role: As our Live-in Care Coordinator, you’ll be designated link between our clients, carers, and management. You’ll ensure that care packages are well written, that carers are well-matched, schedules run smoothly, and both clients and carers feel supported.

· Contributing to the live-in care placements and ongoing rotas
· Matching carers to clients based on needs and personality
· Supporting carers with regular check-ins, problem-solving and unannounced spot checks
· Building effective relationships with families and professionals to ensure continuity of care
· Maintaining accurate records and updating care plans
· Participating in the on-call rota (shared with the team)
· Completing needs and risk assessments for new and existing clients.
· Accurately record information on Careline Live, including evidence of conversation that have taken place.
