Work Shift Day (United States of America) Job Summary: The living donor navigator is a member of the kidney transplant team who will work with prospective transplant candidates and living donors. Their goal is to provide support to candidates by helping them identify and utilize their own Living Donor Champions to seek and engage potential living donors. The living donor navigator will work closely with living donor candidates to educate and improve overall donor comfort with the process, serve as a liaison for any questions or concerns throughout the donor evaluation, provide resources and education to living donors and serve as on-site guides throughout evaluation testing. The position will assist physicians and nurses with moving living donors through the continuum of care by scheduling appointments, making appropriate referrals and navigating criteria based scheduling. Core Responsibilities and Essential Functions: Support living donor candidates throughout all phases of donation Provide donors with education and resources specific to donor needs Provide guidance to transplant candidate and candidates support system to identify living donor champion(s) Assist with marketing to include social media and media coverage Guide donors through onsite evaluation testing Assist with moving live donors through the evaluation process offsite Assist donors with aid applications Schedule patient appointments/ consults Performs other duties as assigned Complies with all Wellstar Health System policies, standards of work, and code of conduct. Required Minimum Education: High School Diploma General or GED General or Associates General-Preferred Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated. Additional License(s) and Certification(s): Required Minimum Experience: Minimum 2 years healthcare setting, patient scheduling or care coordination.
