This is an additional clinical pharmacist post within the practice and we are seeking an experienced Clinical Pharmacist Practitioner with sufficient skills, enthusiasm and commitment to fulfil and develop the role within our practice team. You will work autonomously with an extended role (within own competency) providing a combination of on the day appointments, alongside medicines management, chronic disease management and pharmaceutical advice to the practice population. You will be responsible for seeing and advising patients in respect of their continuing medical and Pharmaceutical needs and developing your skills to see and treat patients who wish to be seen on the day by a health care professional; initially diagnosing and treating patients presenting with minor illness. The Pharmacist practitioner will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.
Patients with problems needing referral to secondary care will be discussed with a GP before making a referral
