Responsibilities

ABOUT THE ROLE

We’re working with local authorities across London to fill essential roles in adult frontline services, including:

  • Hospital Discharge & Community Recovery
  • Mental Health Services
  • Adult MASH & Safeguarding
  • Assessment & Review Teams
  • Learning Disabilities & Autism Support

WHAT YOU’LL BE DOING

Depending on your skillset, your role may involve:

  • Managing caseloads across hospital discharge, community or safeguarding teams
  • Undertaking Care Act assessments and Mental Capacity Act reviews
  • Coordinating discharge planning in collaboration with NHS and MDTs
  • Supporting adults with learning disabilities to achieve independence
  • Working in multi-agency MASH teams to respond to safeguarding referrals
  • Contributing to personalised care and support plans, reviews, and risk assessments
