Job Description: Per Diem Heart Failure Advanced Practitioner –Heart Failure Clinic (outpatient) BILH Lahey Hospital and Medical Center Department of Cardiovascular Medicine is seeking a skilled and highly motivated NP or PA to work in a per diem role within our busy, well-established, Heart Failure Clinic. As an Advanced Practitioner in the Heart Failure Clinic, you will play a key role in the management and care of patients with complex heart failure. You will be responsible for assessing heart failure patients within the clinic setting and developing a comprehensive treatment plan to ensure optimal outcomes. This role requires a compassionate and hardworking individual with a strong foundation in cardiology and, specifically, exceptional knowledge of advanced heart failure care. Responsibilities: · Perform comprehensive patient assessments and diagnostic evaluations related to heart failure. · Develop and implement individualized care plans, including pharmacologic and non-pharmacologic interventions. · Monitor patient progress, adjust treatment plans as needed, and provide ongoing education to patients and families. · Collaborate with cardiologists, nurses, and other healthcare professionals to provide coordinated care. · Order and interpret diagnostic tests, including echocardiograms, EKGs, and lab work. · Provide patient education regarding heart failure management, lifestyle modifications, and medication adherence. · Document all patient interactions and maintain accurate and up-to-date medical records. Qualifications: · Current certification as a Nurse Practitioner (NP) or Physician Assistant (PA). · At least 2 or more years of experience in outpatient cardiology setting required. · Previous experience in heart failure management strongly preferred. · Strong clinical knowledge and the ability to manage complex cardiac conditions. · Excellent communication skills and the ability to work well within a multidisciplinary team. · Compassionate, patient-focused attitude with a commitment to providing high-quality care. Pay Range: $56.25 - $105.00 The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law. Responsibilities
