Shift Availability: Full Time PM Shift (64 hours/pay period) Job Responsibilities: Provides professional nursing care to assigned residents and other areas as needed. Verifies Physician Orders and medication accuracy. Assists in developing resident care plans. Conducts routine rounds, noting and documenting any changes in resident condition (physical, emotional, social, spiritual) electronically. Prepares residents for admission, transfer, or discharge. Instructs residents and families in self-help/self-care. Supervises and directs assigned C.N.A.'s. Maintains resident rights and immediately reports suspected violations per policy. Administers medications and treatments as ordered by physicians. Assesses individual resident needs. Plans, implements, and evaluates care for a defined group of residents. QualificationsRequirements: Must be currently licensed to practice in the state of Wisconsin as a Licensed Practical Nurse (LPN or LVN). Experience working in Assisted Living or Long-Term Care is beneficial, but not required. We will train the right fit for our team! Must be certified in C.P.R. or have the ability to obtain certification within 6 months.
