JOB SUMMARY

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. REQUIREMENTS

  • Valid LPN license in the state of practice.
  • Previous nursing home experience or hospital experience preferred.
  • Familiarity with acute care practices and pediatrics is a plus.
  • Experience in home care, assisted living, senior care, or hospice care is highly desirable.
  • Strong knowledge of medical terminology and medication administration protocols.
  • Proficient in using electronic health records systems (Epic or Cerner).
  • Excellent communication skills with the ability to work effectively in a team environment.

Responsibilities
  • Provide direct patient care, including administering medications and treatments as prescribed.
  • Monitor patients’ vital signs and report any changes to the supervising nurse or physician.
  • Assist with daily living activities such as bathing, dressing, and feeding patients.
  • Maintain accurate patient records and documentation using electronic health record systems like Epic or Cerner.
  • Collaborate with healthcare team members to develop and implement individualized care plans for patients.
  • Perform phlebotomy and laboratory tests as needed, ensuring proper collection and handling of specimens.
  • Educate patients and their families on health management, medication administration, and disease prevention strategies.
  • Support patients with dementia care, pain management, hospice care, and other specialized needs as required.
  • Participate in case management activities to ensure continuity of care across different settings.
