JOB SUMMARY

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including nursing homes, hospitals, and home care environments. The LPN will work under the supervision of registered nurses and physicians, assisting in the delivery of patient care and ensuring that patients receive the best possible treatment.

REQUIREMENTS

• Valid LPN license in the state of practice.
• Previous nursing home experience or hospital experience preferred.
• Familiarity with acute care practices and pediatrics is a plus.
• Experience in home care, assisted living, senior care, or hospice care is highly desirable.
• Strong knowledge of medical terminology and medication administration protocols.
• Proficient in using electronic health records systems (Epic or Cerner).
• Excellent communication skills with the ability to work effectively in a team environment.

• Provide direct patient care, including administering medications and treatments as prescribed.
• Monitor patients’ vital signs and report any changes to the supervising nurse or physician.
• Assist with daily living activities such as bathing, dressing, and feeding patients.
• Maintain accurate patient records and documentation using electronic health record systems like Epic or Cerner.
• Collaborate with healthcare team members to develop and implement individualized care plans for patients.
• Perform phlebotomy and laboratory tests as needed, ensuring proper collection and handling of specimens.
• Educate patients and their families on health management, medication administration, and disease prevention strategies.
• Support patients with dementia care, pain management, hospice care, and other specialized needs as required.
• Participate in case management activities to ensure continuity of care across different settings.
