Description Mountain Community Health is seeking a compassionate Licensed Practical Nurse (LPN) to serve as a Care Coordinator within our interdisciplinary primary care team. This role focuses on building relationships with patients over time — helping them manage chronic medical conditions and substance use treatment as part of fully integrated care. This is not a traditional clinic or primary triage role. The LPN Care Coordinator manages a defined patient panel, provides ongoing follow-up, coordinates care across services, and helps patients stay engaged in treatment and achieve better health outcomes. What You’ll Do Manage an assigned panel of patients and provide ongoing follow-up and support and guidance as they work towards their health goals Monitor health status, treatment adherence, and clinical stability - helping patients stay on track and intervening early when needs change Coordinate care across primary care, behavioral health, and community services to ensure patients receive seamless, whole person support Support patients after hospital or emergency visits and helping them transition safely to everyday life Reinforce care plans and help address barriers to care Collaborate with providers, RN Care Coordinator, and the full care team contributing your clinical expertise to ensure excellent patient outcomes Provide limited protocol-based triage when scheduled Why Join Mountain Community Health: Mission-driven, team-based environment M-F workdays - No weekends, holidays, or evenings Integrated medical, behavioral health, and substance use care model Meaningful patient relationships and long-term impact Supportive, collaborative culture Requirements Active Vermont Licensed Practical Nurse (LPN) license 2+ years clinical experience preferred (primary care, community health, substance use treatment, chronic disease management) Strong communication and documentation skills Experience with care coordination or population health preferred Electronic health record proficiency required
