Description

DESCRIPTION:

Wellbox is a quickly growing healthcare company dedicated to helping people live healthier, happier lives. We do this by providing comprehensive preventative care solutions delivered by an elite team of experienced nurses. Our clinical team, comprised mostly of remote LPNs, works tirelessly to create positive healthcare experiences, empowering people in their health journeys, and improving clinical outcomes for our patients.
Without our incredible team of LPNs, we wouldn’t be able to have the positive impact on providers and patients that we have proven to since 2016. We believe that consistent support, encouragement, and personalized care for our patients truly makes a difference to their health outcomes. If this mission resonates with you, we invite you to join us.

We are seeking tech-savvy, LPN Patient Care Coordinators to conduct monthly telephonic outreach to a panel of chronically ill patients, developing and delivering personalized care plans that address their health concerns and challenges. We work in partnership with healthcare systems and practices to provide these services to their patient base and strive for care continuity and engagement.
We are proud to partner with an amazing team of LPN nurses who guide our patients to better health and help them achieve a greater quality of life. With every interaction, Patient Care Coordinators help close care gaps and increase access to care to those who need it most.
