Overview $2,500 Sign-on Bonus! Citizens Medically Frail Intermediate Care Facility (ICF) provides enhanced medical support to those in need of 24-hour nursing services along with habilitative services, psychology, physical therapy, speech therapy, occupational therapy, nursing, nutrition and recreation services. Residents with chronic medical conditions are able to remain in the community while receiving enhanced medical supports and reduce disruptive and restrictive hospital stays. Responsibilities Primary duties and responsibilities include but not limited to: Maintains optimal health care for ICF individuals by administering medication and medical treatments, and monitoring health status Coordinates the delivery of primary and specialized medical services under the direction of the RN, and serves as medical services advocate for each individual Practices and promotes the agency’s mission statement in every interaction Provide ongoing support to people in becoming active participants in every aspect of their lives and to ensure that a high quality of life is maintained Physical Demands/Work Environment: Physical aspects may include but are not limited to lifting and transferring of an adult/child participant Qualifications Requirements: Must be a NYS licensed practical nurse. CPR, fire safety training, and medication knowledge is required
Responsibilities
The LPN is responsible for maintaining optimal health care for individuals in the ICF by administering medications and monitoring health status. They also coordinate medical services and advocate for the residents' medical needs.
