Job Details Job Location: ORANGE, CT 06477 Position Type: Full Time Salary Range: $34.00 - $36.00 Hourly Travel Percentage: None The LPN Clinical Team Assistant will be responsible for providing clinical, clerical and administrative support necessary to ensure efficient delivery of care for the patients and staff of the home care interdisciplinary team. Essential Functions Assist in HHA orientation at the direction of the RN. Monitor RN supervisory visit compliance via scheduling procedures. Performs patient centered scheduling duties, utilizing the scheduling system, and assigns cases to appropriate interdisciplinary team members. Ensures visits are scheduled for patients appropriately, within agency standards for initiation of services, and maintaining appropriate coverage for patient visits. Tracks and reviews field staff schedule daily and adjusts as necessary to drive productivity and LPN usage. For NP visits (f2f), check and verify / assign f2f visit for pt that are in the certification window ongoing into 3rd benefit period. IDT Prep: assign the pt’s to the list, update the curriculum and send out to staff for IDT Handles Pop audits: audit and track the soc paperwork and update tracking sheets accordingly. Ensure field notes are timely. Review DME and pharmacy utilization for cost effectiveness and appropriateness. Maintains effective communication with field staff and supervisors. Handles complaints including documentation and follow-up. Assist supervisor with calls and other administrative tasks as needed assigned. Performs patient centered clinical support for supervisors and clinical staff to include: Answers and Triages calls from field staff, patient/ families, and medical offices. Coordination of care between disciplines as directed by RNCM. Medical documentation recording all interactions. Problem solving clinical, administrative and customer service issues. Qualifications Qualification Requirements Graduate of an accredited LPN program. The employee has the required licensure, education, training, and qualifications required by rule, regulation, law and/or statute to qualify for the role in their state. Minimum of 2-3 years previous office/scheduling experience preferably in a Healthcare Environment. Medical terminology knowledge and ability to research new terms. Computer experience includes proven ability to learn a computerized system. Previous experience with excel and word. Excellent communication and organizational skills. Ability to handle multiple tasks, work at a fast pace with attention to detail and accuracy. Excellent interpersonal and customer services skills
