Description Job Title: Licensed Practical Nurse Reports To: Director, Community Day Services Supervises: As Assigned General Description The Nurse at the Joseph Koenig Sr. Training Center and Lois Lloyd Center serves as a key resource for medical and healthcare-related issues, overseeing the medical needs of individuals served. This role ensures the implementation of health procedures and compliance with organizational policies. Qualifications Education & Licensure Licensed Practical Nurse with a valid Illinois Professional Nursing License. Experience Minimum of one year of experience working with individuals with intellectual and developmental disabilities or experience in community nursing. Abilities & Skills Strong analytical and problem-solving skills in healthcare planning and evaluation. Proficiency in English reading, writing, and verbal communication. Strong interpersonal, math, computer literacy, organizational, and driving skills. Ability to work independently and collaboratively within an interdisciplinary team. Essential Duties & Responsibilities 1. Medical Oversight & Individual Care Monitor and maintain medical records for individuals served. Order and manage inventory of medical supplies. Track absences related to medical concerns. Participate in Personal Plan meetings and provide relevant medical data. Refer individuals and families to medical facilities, doctors, and dentists as needed. Provide health-related consultation to families and caregivers. Develop medically appropriate toileting procedures with staff. Conduct training sessions on health and personal care topics. 2. Medical Resource & Health Coordination Provide first aid treatment for individuals and staff as needed. Develop and implement health and medical procedures. Arrange and participate in health screening programs. Ensure compliance with external healthcare regulations and standards. Alert leadership to potential health-related concerns. Conduct in-service training for staff on health and medical issues. Prepare medical assessments and reports as required. 3. General Responsibilities & Compliance Adhere to all organizational policies, procedures, and guidelines. Maintain confidentiality and advocate for the rights of individuals served. Emphasize safety protocols, including Universal Precautions. Recommend and implement risk prevention strategies. Report suspected misconduct, including fraud, abuse, or neglect, in compliance with regulations. Participate in training for skill development. Maintain an organized, clean, and functional workspace. Serve on committees or represent the organization as assigned. Supervise volunteers and interns as required. Requirements Physical Requirements Ability to sit, bend, lift, push, pull, carry, and maintain independent mobility. Perform light maintenance and cleaning as needed. Assist in lifting or carrying boxes and packages. Special Equipment & Clothing Closed-toe shoes with non-slip soles required. Recommended: Minimal jewelry (avoid earrings, raised rings, bracelets, chains, or pins).
